Physical exam shows pale mucous membranes, prolonged CRT, tachycardia (180/min), and a tympanitic cranial abdomen. Which is the most appropriate next step?
The presentation is classic for gastric dilatation-volvulus (GDV) with shock. The priority is stabilization (large-bore IV catheters, rapid isotonic crystalloids ± hypertonic saline/colloids as indicated), immediate gastric decompression (orogastric tube if possible or trocarization), analgesia, and then definitive surgical correction with gastropexy. Delaying for imaging risks death. Inducing emesis is contraindicated.
