A 9-year-old male neutered Labrador Retriever is presented for acute onset nonproductive retching and progressive abdominal distension after eating. The dog is restless, hypersalivating, and has pale mucous membranes. Heart rate is 180/min, CRT is 3 seconds, and pulses are weak. An orogastric tube cannot be passed. Findings are classic for GDV with shock (retching, distension, inability to pass tube, tachycardia, poor perfusion). Stabilization requires immediate decompression (trocarization if tube cannot pass) and aggressive IV fluids before definitive surgery. Delaying for imaging or inappropriate therapies worsens outcome.
