On exam, the dog is restless, has pale mucous membranes, CRT 3 seconds, HR 180/min, and a tympanic cranial abdomen. An orogastric tube cannot be passed. Which is the most appropriate next step?
The presentation is classic for GDV with shock. Immediate stabilization and gastric decompression are indicated; percutaneous trocarization can rapidly relieve pressure when a tube cannot be passed, while aggressive IV fluid resuscitation is initiated. Imaging should not delay life-saving stabilization. Definitive treatment is surgical gastropexy once stable.
