A 6-year-old neutered male Labrador Retriever is presented for acute onset abdominal distension and nonproductive retching. The dog is restless and hypersalivating. On exam, HR 180/min, pale mucous membranes, CRT 3 seconds, and a tympanic cranial abdomen. You place two large-bore IV catheters and begin isotonic crystalloid boluses. Which additional step is most appropriate to perform immediately while preparing for surgery?

This presentation is classic for GDV with shock. After initiating aggressive IV fluid resuscitation, immediate gastric decompression (orogastric tube if possible; otherwise percutaneous trocarization) is indicated to improve venous return, reduce pain, and stabilize the patient prior to anesthesia and definitive surgical gastropexy. Waiting for radiographs can worsen shock; emesis is contraindicated; activated charcoal and diuretics are not appropriate.
