A 6-year-old neutered male Labrador Retriever is presented for acute onset abdominal distension and nonproductive retching. The dog is restless and hypersalivating. On exam, HR 180/min, pale mucous membranes, CRT 3 seconds, and a tympanic cranial abdomen. You place two large-bore IV catheters and begin isotonic crystalloid boluses. This presentation is classic for GDV with shock. After initiating aggressive IV fluid resuscitation, immediate gastric decompression (orogastric tube if possible; otherwise percutaneous trocarization) is indicated to improve venous return, reduce pain, and stabilize the patient prior to anesthesia and definitive surgical gastropexy. Waiting for radiographs can worsen shock; emesis is contraindicated; activated charcoal and diuretics are not appropriate.
